Application to Expedite a Lost or Stolen US Passport

Step 1: Complete DS-11 application form and DS-64 application form (if lost or stolen)
The application form required for this process is the DS-11 application form. This form is available online and it must be completed online. Make sure that upon completion, you do not sign the form at the end. The form will be signed in front of an Acceptance Agent who will verify your documents first.
Compared to other passport application processes, an additional form will be required to report it as either a lost or stolen passport. The application form is called DS-64. This form should be completed online and unlike the DS-11 application form, it should be signed and dated.
Step 2: Gather all required documents
Once you have completed the DS-11 application form and DS-64 application form, following is the list of documents you must prepare before you start your application process:
- Completed DS-11 Application form
- Completed DS-64 Application form for lost/stolen passport
- Proof of Identity (a valid driver’s license, Government Issued photo I.D).
- Please call for a Letter of Authorization.
- Travel itinerary.
- Certified Check or Money Order to “US Department of State” for the appropriate amount.
Step 3: Getting the U.S. Passport requirement documents authorized.
For the completion of this step, you will be required to locate the nearest Acceptance Agent office. The Acceptance Agent will conduct a document authorization procedure. The following consist of the Acceptance Agent’s authorization procedure:
- The Agent will act as a witness to the signing of the application form DS-11 which was left unsigned in Step 1
- The Agent will collect $35.00 execution fee and check the applicant’s ID
- The Agent will verify the documents that you have submitted
- The Agent will then seal the documents into an envelope.
The Acceptance Agent should write the applicant’s name, departure date, and date of birth on the outside of the sealed envelope. If two or more applications are sealed in the same envelope please make sure that the information about both applicants is written on the envelope.
ALL MINORS MUST APPEAR IN PERSON.
You must inform the Agent that you are using a passport agency to expedite the processing and that the sealed documents will be hand-delivered to the Department of State. This Department of State policy is called “Hand Carrying of Executed Applications.” The policy is detailed on page 31 and 44 of the Acceptance Agent Manual.
The Acceptance Agent will then return the sealed envelope to you. Do not open the envelope. FedEx or bring the sealed envelope and the rest of the documents to our office for processing.
